It is not really like that anymore, but people do have whole swathes of vocabulary that outsiders do not understand. I think in France you got rid of the diversity in a lot of ways by having the French Language Academy.

Yes, we have a few different accents (and not a lot) but the world are basically the same, except for a few.

You do not expect to not understand someone in France, it may just be more difficult because of the accent.
